Application for Funding

SFY 2023

Alcohol, Drug Addiction, & Mental Health Services of Montgomery County (ADAMHS) will be releasing the Application for Funding (AFF) for program proposals to be implemented from July 1, 2022, through June 30, 2023. Applications are being accepted for prevention and early intervention of substance use and other mental health concerns, and professional educational/workforce development programs in partnership with academic institutions. Proposals should address the behavioral health needs in the Montgomery County community and align with the Montgomery County priority populations identified in the ADAMHS Community Plan.

Those interested in applying for funding must meet the applicable funding guidelines and complete an online application. Please click the links below to review the documents regarding the Application For Funding. 


ADAMHS reserves the right to reject proposals and/or fund less than the requested amount. Any entity that receives ADAMHS funding will enter a contract with ADAMHS prior to any payment of allocated funds. All decisions of ADAMHS on the allocation of funds are final and are contingent upon the receipt of allocations from the Ohio Department of Mental Health and Addiction Services (OhioMHAS).

All requestors must meet at least one of the following criteria:

  • Hold OhioMHAS licensure/certification for prevention services
  • Serve as an Education entity under the jurisdiction of the Ohio Department of Education or the Ohio Board of Regents
Applications shall address all requirements and shall contain the full name of each agency/company and shall be signed by officials authorized to execute a contract. 

Thank you to those of you that reported issues with the Application for Funding portal. Due to technical issues with the portal and the upcoming deadline for the Application for Prevention Services Funding fiscal year 2023 approaches, we have created the outlined process below required for all submissions.

Only applications submitted through the following processes by April 7th, 2022, at 5 PM will be considered for funding recommendations. There will be two options provided for submission.
Option One.

There are two required forms that entail a complete application, each agency must complete all required components as outlined in the Application for Funding guidelines provided (a copy is attached for reference). The questions within both forms are the same as those within the Application for Funding Portal that was shared in all previous communications.

Organizational Information Form

  • Each agency will be required to turn in only ONE copy of this form.
  • Please be sure to use the attached templates for all assurance documents required on this form.

Programs & Services Form

  • An individual form must be filled out for each proposed program. The same link can be used to submit each program and its related services.
  • Example: If you have three proposed programs, the Programs & Services form must be submitted three times.
  • A separate budget must be submitted for each proposed program. The required budget template is attached.

Option Two.

  • There are two required components that entail a complete application, each agency must complete all required components as outlined in the Application for Funding guidelines provided (a copy is attached for reference).  
  • Organizational Information
  • Each agency will fill out all sections in the Application for Funding Portal except for the Programs & Budgets sections that will be submitted in the Program and Services google form.
  • Programs & Services Form

Additional Information

  • All funded programs will be required to report outcomes to ADAMHS per contract
  • All AFFs are reviewed by ADAMHS Recommendations for funding are reviewed by the Program and Services Committee and/or the Executive Committee. The decision to fund a program rest with the ADAMHS Board of Trustees.
